About the dengue test

Dengue is a seasonal certainty in Bangalore, and the tests are chosen by how many days the fever has run. NS1 antigen is the early test — reliable roughly from day one to day five. IgM and IgG antibodies appear later, generally after day five. The part that matters most clinically, though, is not the diagnosis but the monitoring: platelet counts fall over the following days, and the danger window is typically around the time the fever settles. Daily or alternate-day CBCs are standard, and having a nurse come to the house for those repeat draws — rather than moving a dehydrated, aching patient to a lab every morning — is genuinely safer and is one of our most-requested services in monsoon months.

Which dengue test should be done, and when?

It depends on how long the fever has lasted. NS1 antigen is the test for the first five days and is the one to ask for early. IgM antibody becomes useful from about day five onwards, and IgG indicates past or secondary infection. If you are unsure, tell us which day of fever the patient is on and we will collect what fits — or simply send a photo of the doctor's prescription.

Can you come daily to check platelet counts?

Yes, and this is one of our most-used services in the monsoon. Confirmed dengue patients typically need daily or alternate-day CBCs for several days, and moving a weak, dehydrated patient to a lab each morning is both miserable and avoidable. Our nurse comes to the house, draws the sample, and can also record vitals and watch for warning signs like bleeding, severe abdominal pain or drowsiness.

At what platelet count is dengue dangerous?

There is no single number that decides this, and it is genuinely a doctor's judgement rather than a threshold you should act on yourself. Counts below one lakh warrant close monitoring, and the risk period is often around the time the fever settles rather than at its peak. Warning signs — bleeding, persistent vomiting, severe abdominal pain, drowsiness — matter as much as the count. If any of those appear, seek medical care immediately rather than waiting for the next report.
