About the typhoid (widal) test

Typhoid remains common in India and typically presents as a fever that climbs over days rather than spiking suddenly, along with headache, abdominal discomfort, loss of appetite and marked weakness. The Widal test looks for antibodies against Salmonella typhi, and it has an important timing quirk: antibodies take about a week to appear, so a Widal done on day two of fever is frequently negative even in genuine typhoid. Blood culture is the more definitive test, especially in the first week. Because typhoid patients are usually too weak and febrile to travel, home collection is the practical way to get this done.

When should the Widal test be done during fever?

Not too early. Widal detects antibodies, and antibodies take roughly a week to develop — so a test done on day one or two of fever is often negative even when the person genuinely has typhoid. Most doctors want it from about the fifth to seventh day of fever, and in the first week they prefer a blood culture instead, which detects the organism itself rather than the immune response to it.

Is the Widal test reliable on its own?

It has real limitations and should not be read in isolation. Titres can stay elevated for months after a past infection or a typhoid vaccination, and in areas where typhoid is common a fair share of the healthy population carries a baseline titre. That is why doctors interpret it against the clinical picture, alongside a CBC, and confirm with blood culture where it matters. Please do not self-diagnose from a Widal report.

Can you collect a sample from a patient with high fever at home?
