About the liver function test

A liver function test measures the enzymes and proteins that reveal how well the liver is working and whether its cells are being damaged. It is ordered for jaundice, for persistent fatigue and loss of appetite, for anyone on long-term medication that is processed by the liver — anti-tubercular drugs, statins, anti-epileptics, methotrexate — and for regular alcohol users. Fatty liver disease is now extremely common among urban Indian adults, frequently silent, and often first spotted as a mildly raised SGPT on a routine health check rather than through any symptom.

Frequently asked about the liver function test

Is fasting required for a liver function test?

Most laboratories and doctors prefer an 8–10 hour fasting sample, especially when the LFT is being done alongside a lipid profile or fasting sugar as part of a health check. Water is allowed. Just as important: no alcohol for at least 24 hours beforehand, because even social drinking the previous evening can lift GGT and SGPT enough to produce a misleading report.

What does a high SGPT mean?

SGPT (also written ALT) is the enzyme most specific to liver cells, so a raised level means liver cells are being irritated or damaged. In urban India the commonest reason by far is fatty liver, followed by alcohol, viral hepatitis and drug effects. A mild rise is common and frequently reversible with weight loss and stopping alcohol; a marked rise needs proper evaluation. Only your doctor can tell which is which — do not self-interpret this value.

Can you collect an LFT sample for a jaundice patient at home?

Yes, and it is a sensible thing to do. A jaundiced patient is usually weak, nauseated and advised rest, so bringing them to a lab is both uncomfortable and unnecessary.
