About the thyroid profile test

Thyroid disorders are strikingly common in India — hypothyroidism especially, and disproportionately in women — and the symptoms are so ordinary that they get dismissed for years: tiredness, weight gain, feeling cold, hair fall, dry skin, low mood, irregular periods, difficulty conceiving. The thyroid profile settles the question with three numbers. TSH is the most sensitive of them and usually moves first, which is why some doctors order TSH alone as a screen and add T3 and T4 when it is abnormal. Anyone already on thyroxine needs repeat testing every 6–12 weeks until stable, and this ongoing monitoring is where home collection saves the most time.

Frequently asked about the thyroid profile test

Do I need fasting for a thyroid test?

No — T3, T4 and TSH do not require fasting, and the sample can be taken at any time of day. There is one rule that genuinely matters though: if you are already on thyroxine, the sample must be drawn before you take the morning tablet. Testing an hour or two after a dose falsely inflates T4 and can lead your doctor to reduce a dose you actually need.

Is TSH alone enough, or do I need the full T3 T4 TSH profile?

For a first screen in someone with vague symptoms, many physicians order TSH alone, because it is the most sensitive marker and moves before T3 and T4 do. The full profile is preferred when TSH comes back abnormal, when symptoms are strong, during pregnancy, and for anyone already on treatment. Why is thyroid testing important in pregnancy?

Untreated hypothyroidism in pregnancy is associated with real risks to both mother and baby, and the acceptable TSH range in pregnancy is tighter than the ordinary adult range. Most obstetricians screen in the first trimester and repeat through the pregnancy for women already on thyroxine, since the required dose usually rises. Your obstetrician sets the target — pregnancy results should never be judged against a standard adult reference range.
