About the esr test

ESR measures how quickly red blood cells settle at the bottom of a tube in one hour. A raised ESR is a general signal that inflammation is present somewhere in the body — it doesn't say where or why, which is why doctors almost always read it together with a CBC and CRP. It is widely used in India to follow up tuberculosis, rheumatoid arthritis, chronic infections and unexplained long-standing fever, and to track whether treatment is working over weeks and months.

Is fasting required for an ESR test?

No. ESR needs no fasting and no special preparation. Eat and drink normally, and take your regular medicines unless your doctor has said otherwise.

What does a high ESR mean?

A raised ESR means inflammation is present somewhere in the body — it does not identify the cause on its own. It can rise in infection, arthritis, tuberculosis, autoimmune disease, and even in pregnancy or with anaemia and increasing age. This is precisely why it is never read in isolation: your doctor will interpret it alongside your CBC, CRP, symptoms and examination.

Is ESR the same as CRP?

They overlap but they are not the same. CRP rises and falls quickly — within hours — so it reflects what is happening right now, which makes it better for acute infection. ESR moves slowly over days and weeks, which makes it better for tracking chronic conditions over time. Doctors frequently order both together for exactly this reason, and we collect both from a single draw.
