About the hormone & fertility test

Fertility and hormone testing is unusually sensitive to timing, and that is where most home collection services fall down. LH, FSH and oestradiol are typically drawn on day two or three of the menstrual cycle; progesterone is drawn about a week before the next period is due; prolactin should be taken in the morning, at rest, since stress and even a difficult blood draw can raise it. AMH is the exception — it can be taken on any day of the cycle. Our nurses know these rules and will schedule the visit for the right day rather than simply collecting whenever is convenient, because a hormone drawn on the wrong day produces a number your gynaecologist cannot use.

Which day of my cycle should hormone tests be collected?

It depends on the hormone, and getting it wrong makes the result unusable. LH, FSH and oestradiol are usually drawn on day two or three of your period, counting the first day of full flow as day one. Progesterone is drawn around day 21, roughly a week before the next period. Prolactin should be a morning sample taken at rest. AMH can be done on any day. Does AMH need to be done on a particular day?

No — AMH is the convenient one. It stays relatively stable through the cycle, so it can be collected on any day and at any time, without fasting. That is part of why it has become such a widely used marker of ovarian reserve. Its interpretation is strongly age-dependent, so the number means little without your gynaecologist reading it against your age and history.

Because prolactin genuinely responds to stress, exertion and even the anxiety of a blood draw. A rushed trip through traffic followed by a wait in a crowded lab can lift the reading enough to prompt a repeat test.
