About the uric acid test

Uric acid is the waste product left when the body breaks down purines from food and from its own cells. When levels stay high, crystals form in joints — producing the sudden, severe, unmistakable night-time pain of gout, most classically in the base of the big toe — and in the kidneys, where they form stones. Levels are influenced by red meat, seafood, alcohol (beer especially), certain diuretics, and declining kidney function, which is why uric acid is usually read together with a kidney function test rather than alone.

Do I need to fast for a uric acid test?

Not strictly, though many doctors prefer a fasting morning sample for consistency. What matters more is avoiding alcohol and a heavy meat or seafood meal in the 24 hours before — both push the level up temporarily and can turn a normal result into a falsely raised one.

Does high uric acid always mean gout?

No. Plenty of people have raised uric acid and never develop a single attack, and conversely some people have a genuine gout attack while their blood level reads normal — during an acute attack, urate can shift out of the blood into the joint. Gout is diagnosed on the clinical picture, not on the number alone. Your doctor interprets the result against your symptoms.

Which tests are usually ordered with uric acid?

Kidney function tests, most often — because impaired kidneys are one of the commonest reasons uric acid rises, and because urate-lowering drugs are dosed according to kidney function.
