About the iron studies test

Iron-deficiency anaemia is the commonest nutritional deficiency in India, and it affects women of reproductive age most of all. A CBC will show that anaemia exists; iron studies explain why. Ferritin is the key number because it reflects stored iron and falls before haemoglobin does — meaning iron deficiency can be identified and corrected before someone becomes visibly anaemic. This panel is also used to monitor patients on oral iron or after an iron infusion, both of which are common in our post-surgical and post-partum caseload.

Frequently asked about the iron studies test

Should I stop iron tablets before this test?

Usually yes — but only with your doctor's agreement. Oral iron raises serum iron for many hours after a dose, so a sample taken the same morning measures the tablet rather than your actual stores. Most doctors ask patients to hold iron for 24–48 hours before the test. Never stop a prescribed medicine on your own; confirm with the prescribing doctor first.

Why do I need iron studies if my CBC already showed anaemia?

Because a CBC tells you that anaemia exists, not why. Anaemia can come from iron deficiency, B12 or folate deficiency, chronic disease, kidney disease, thalassaemia or blood loss — and the treatments are completely different. Giving iron to someone whose anaemia is not iron-related does nothing and delays the real diagnosis. Iron studies are what separate these possibilities.

Can ferritin be low even when haemoglobin is normal?

Yes, and this is a genuinely useful thing to know. Ferritin reflects stored iron and drops first; haemoglobin falls only once those stores are exhausted. So a person can have depleted iron stores — with real fatigue, hair fall and restless legs — while their haemoglobin still reads normal. Catching it at that stage is far easier to correct.
