About the calcium test

Calcium in the blood is tightly controlled by the parathyroid glands, vitamin D and the kidneys, so an abnormal level is usually a signal about one of those systems rather than about diet. Low calcium produces muscle cramps, spasms and tingling around the mouth and fingers; high calcium causes fatigue, constipation, excessive thirst and kidney stones. It is a standard part of osteoporosis assessment in post-menopausal women, of kidney disease follow-up, and of the workup for recurrent kidney stones — and it is nearly always read alongside vitamin D.

Frequently asked about the calcium test

Does a normal blood calcium mean my bones are fine?

No — and this is a common misunderstanding. Blood calcium is held in a narrow range by the body even when bones are losing mineral, because the body will draw calcium out of bone to keep the blood level stable. Bone density is assessed by a DEXA scan, not by a blood test. Serum calcium is still worth doing, but read it alongside vitamin D and your doctor's overall assessment.

Is calcium usually tested with vitamin D?

Very often, yes, because the two are physiologically linked — vitamin D governs how much calcium you absorb from food. Doctors investigating bone pain, cramps or osteoporosis typically order both together, and sometimes phosphorus and PTH as well. We collect all of these from a single draw in one visit.

Do I need to fast for a calcium test?
